Individualized Programming – A Case Manager coordinates communication between families/guardians, staff and other agencies. Through this communication, an individualized program plan is formed. This plan reflects the strengths, preferences, and needs of the person served and is designed to maximize independence, productivity and integration into the community. Individual plans are reviewed every six months, or on an as-needed basis, to ensure that needs of the individual are being met.
